Harm happens - now what? The life cycle of a patient safety incident

Improving health care is hard work. This workshop will provide you with an understanding of various patient safety tools that, when used together, address the continuum of recognizing, responding to and learning from harm. To further support this concept of a continuum, you will have an opportunity to participate in an interactive case study that spans the life cycle of a patient safety incident.
At the end of this course you will be able to
- Articulate the importance of a provider team debriefing following a patient safety incident.
- Recognize patient and family perspectives when harm happens.
- Develop a flexible approach to the disclosure of a patient safety incident.
- Identify key differences in communicating to the Board and the organization versus patients and families.
- Explain the importance of preparing to inform the public when harm happens.
- Summarize the process for meaningful analysis of a patient safety incident and the development of recommended actions.
